Candidates who have taken the public TOEIC Listening & Reading test with Amideast in the past two years are eligible to order additional TOEIC Listening & Reading score reports.  

Only nursing candidates who need to send their scores directly to CGFNS in Philadelphia, NJ (USA) should use this form.   We are not able to issue refunds for additional score report orders, so please review your order before submitting.
 

Please read all information below carefully. Incomplete/ improperly submitted orders will be cancelled.
 
FEES – TOTAL $50

For nursing candidates, we will include a cover letter with your TOEIC Speaking & Writing Score Certificate, which will be sent by FedEx 2-day service.

 
VERY IMPORTANT INSTRUCTIONS – HOW TO ORDER
Failure to follow these instructions EXACTLY will result in delays.
1.      The name on the profile should be the candidate’s name.  If not, create a new profile.
2.      During checkout, you will be asked for certain information.  Do not skip this.   Verify that the information is accurate.

2A  - Date of your TOEIC LR test, as it appears on your score report.  Remember: we cannot process requests for tests which occurred more than two years ago.

2B - Your testing location
2C - Your Reference #/File #.  This is optional but highly recommended if you have a reference # or file #.  We will add this information to the cover letter which we include with your score report.  This helps CGFNS process your score report more efficiently.
 
CANCELLATION AND REFUND POLICIES
We are unable to issue refunds for Additional Score Report.  Please review your order carefully before submitting!
If we are unable to process your order, your order will be cancelled, and full refund will be issued.
