TOEIC Score reports for Nurses – Additional Score Reports(ASR) for CGFNS

Candidates who have taken the public TOEIC 4 Skills or TOEIC Speaking &Writing + Listening & Reading test with AMIDEAST in the past two years areeligible to order additional TOEIC 4 Skills or TOEIC Speaking & Writing + Listening& Reading score reports.  

Only nursing candidates who need to send their scoresdirectly to CGFNS in Philadelphia, NJ (USA) should use this form.   We are not able to issue refunds foradditional score report orders, so please review your order before submitting.

Please read all information below carefully. Incomplete/improperly submitted orders will be cancelled.

FEES – TOTAL $50

For nursing candidates, wewill include a cover letter withyour TOEIC 4 Skills or TOEIC Speaking & Writing + Listening & Reading ScoreCertificate, which will be sent by FedEx 2-day service.

VERY IMPORTANT INSTRUCTIONS – HOW TO ORDER

Failure to follow these instructions EXACTLY will result indelays.

1.      The name on the profileshould be the candidate’s name.  If not,create a new profile.

2.      During checkout, you willbe asked for certain information.  Do notskip this.   Verify that the informationis accurate.

Additional Information ** required for TOEIC SW score reportorders**

2A - Date of your TOEIC SW test, as it appears on your scorereport.  Remember: we cannot processrequests for tests which occurred more than two years ago.

2B - Your testing location

2C - Your Reference# /File #.  This is optional but highly recommended ifyou have a reference # or file #.  Wewill add this information to the cover letter which we include with your scorereport.  This helps CGFNS process yourscore report more efficiently.

CANCELLATION AND REFUND POLICIES

We are unable to issue refunds for Additional ScoreReport.  Please review your ordercarefully before submitting!

If we are unable to process your order, your order will becancelled, and full refund will be issued.

AMIDEAST can only provide TOEIC scores for exams deliveredin the last two years – no exceptions.
